Please use the Case Management Project Template and add this week’s steps to your existing document. This week, students will establish goals and implement a ‘service intervention plan’ for this client. Be sure to review the four ethical principles (autonomy, beneficence, nonmaleficence, justice, and veracity) as you establish a service plan for your fictitious client.

• Establish 1-3 attainable and realistic goals for your client (1/2 to ¾ page). Goals are what clients should be able to do after case management services are terminated. Example: Substance Abuse Client Goal: Client will be able to identify triggers and apply a new association to help them maintain sobriety.
• Establish a service intervention plan (no more than 2-4 strategies that help client achieve the goals for case management services) Implementation of services should align with community resources and support systems available to each client through their micro, mezzo, and macro systems. This plan should be clear, concise, and easy-to-follow. (no more than 1/2 page). Example: Substance Abuse Client Intervention Strategy: Client will attend a 30-day residential treatment program.
• This portion of the assignment should be no more than two pages double-spaced. APA formatting is not required unless using a source to support your writing.
