You will continue to develop the Case Management Project by incorporating Part Two. This week, you will review feedback from Part One and make any corrections noted by the instructor. You will use the same document and add to it each week. By the end of this course, you will have completed the steps in the case management process for a fake client (their population was determined by you in the first week).
Instructions
- Read Chapter 4 An Example of Case Management (special attention to pp. 42-43). Review Chapter 2 (Steps in the Case Management Process, p.19) with a focus on determining the severity of the problem, developing a hypothesis for clients’ issues, and keeping in mind the many variables associated with this individual’s biography.
- Read Chapter 6 Specialized Practice Skills as this section goes into more detail regarding the case management steps.
- Review instructor feedback from Week 1 & implement any suggested changes.
- Keep in mind the model that you are implementing as it will help further develop this project (refer to p. 14 of the textbook).
- Continue using the Case Management Project Template. Please add the following steps to this project for Week 3. Remember, your writing should be professional and free of any judgmental or bias language.
- Determine the severity of the client’s problem with several supporting statements
- Develop a hypothesis concerning why clients’ problems are occurring. Keep in mind the psycho-social perspective and systems theory as you develop the hypothesis for this client.
- This assignment should be no more than 1 page.
Thinking ahead to Week 4 As you complete this week’s portion of the Case Management Project (Part 2), reflect on the next stage of the process. Good case managers are thinking ahead and being proactive.
