There are two overarching goals for case management. The first goal is to connect clients with the services needed to help them resolve their problem(s). The second goal is to help clients become independent and successfully take care of their problems without the need for intervention from case management services.
The Case Management Project in HSV2300 will take place, in sequential steps, just like the case management project unfolds within the workplace.
Instructions You will use the Case Management Project Template to assist you in drafting an intervention plan for a fictitious client. Each week, you will add to the steps of the case management project until you reach the follow-up phase in Week Five.
- Read: Chapter 2 (Overview of Case Management Practice - specific attention to p. 17-19),
- State the model you are using for this project (see p. 14 for guidance). Case management can fall under these different models. Remember, case managers, work in a variety of different settings and may have varying job functions. What type of role do you have with this fictitious client?
- The first step in the Case Management Project is to identify your client (the population that you will be serving for this fictitious case management service). You are encouraged to identify a population that you currently work with or plan to work with in the future. This project should have special meaning to you; therefore, it is essential for you to be able to select your population. Chapter 9 will provide insight into the various populations serviced by case managers. Remember that we are looking at the person from a psycho-social perspective. Keep in mind systems theory as you identify your demographic and create a brief biography of this individual. In essence, you are creating a fake client for the sake of this project.
- After selecting the population, you will need to ‘identify the problem’ that will be addressed in your Case Management Project. See p. 19 of your textbook. Remember, this week, you are only selecting the population and identifying the problem.
- Include the following in this week’s assignment. Use the Case Management Project Template to format this assignment. You will add to this document each week as we progress to new steps in the case management process. It is located under course documents on your course home page.
- Provide a detailed description of your selected population (you are basically giving a biography of this fictitious client) – one paragraph.
- Identify the problem without using judgmental or bias language. – one paragraph.
Thinking ahead to Week 3: Think about the ‘severity of the problem’ as you will write about this step next week. You will also ‘develop hypotheses concerning why client problems are occurring’. Case managers are always thinking ahead to the next step in the process. You have a few days to reflect on your next steps.
